Columbus CEO is proud to announce the 2015 HealthCare Achievement Awards.
In the March 2015 issue, Columbus CEO magazine will recognize outstanding central Ohio healthcare professionals and organizations for making a difference in the lives of their patients, colleagues and community. Two new awards will be given for the first time!
Awards will be given in these six categories:
Executive of the Year (click to nominate)
This award recognizes an individual for having a positive impact on a healthcare organization and those it serves through sustained, outstanding administrative efforts.
Practitioner of the Year (click to nominate)
This award recognizes an individual (physician, nurse, physician’s assistant, etc.) for providing outstanding care and/or advancing the practice of medicine.
Lifetime Achievement (click to nominate)
This award recognizes a longtime or retired healthcare professional who has demonstrated exceptional leadership, knowledge, decision- making skills and commitment to his or her organization, patients and profession.
Volunteer of the Year (click to nominate)
This award recognizes an individual or individuals for making a positive impact by devoting time, expertise and/or dollars to support local healthcare organizations, patients and the community.
Healthcare Trailblazer—new! (click to nominate)
This award recognizes an organization, company or individual for innovative or entrepreneurial healthcare initiatives that improve the practice of medicine or delivery of care to patients.
Pathway to Population Health—new! (click to nominate)
This award recognizes an organization, company or individual for creative efforts that get in front of healthcare issues to effectively reduce the incidence of disease and improve the well-being of communities
Making a nomination
To nominate a deserving individual or organization, fill out the appropriate form and email it to firstname.lastname@example.org or mail to Columbus CEO at 34 S. Third St., Columbus, OH 43215. Up to three finalists and a winner will be selected in each category. Previous winners are not eligible, but earlier finalists can be nominated again. Nomination materials will be used by the editorial staff to compile stories on the winners and finalists. Winners and finalists may be asked to provide more information or photographs as part of the March 2015 issue. All nominations must be received by Oct. 15.
Choosing the Winners
A special selection committee will review all nominations and choose a group of finalists and winners. Winners and finalists will be notified directly by Columbus CEO at the work address and/or email provided on the nomination form. In addition to appearing in a story in the magazine’s March 2015 issue, winners and finalists will be honored at a special event in early 2015.
